Q. I am a 15 years old boy. About a week ago, I had a hamstring strain. Yesterday, I was running during the softball practice and got severe pain in both my legs. I could barely walk, but I kept on running to finish the practice session. I'm still having the pain in my legs. Is pain in my legs due to pulled hamstring
A.  It is uncommon to have both hamstrings pulled; I think your muscles are not well conditioned enough to take the load of the intense game. Remember, it is important to have good fitness regimen for playing any game as games do not provide all-round fitness. I suggest you take rest for 3-4 days and gradually start the game and follow good fitness routine and do good warm-up and limbering down regularly. Playing a competitive game with pain means inviting a serious injury, so refrain.



When do I start playing football after ligament tear

Q. I am 17 years old male and I recently had a ligament tear in my ankle. I am a football player and want to know when I can start playing again?
A.  Here is a general guideline for return to play activities after ankle sprains:
Pain-free full range of motion: the injured body part should have full movement and flexibility with little or no discomfort.
Return of strength: the injured body part should be approximately equal (90-95 percent) to the opposite side before returning to full activity.
Minimal pain or swelling: some mild discomforts stiffness and/or swelling during or after exercise should be expected during the initial return to activity. This responds well to ice therapy.
Functional retraining: you should be able to perform the specific motions and actions required for your sport effectively before returning to activity.

Q. I am a 26 years old male. I had a minor accident six months back, which injured my right testicle. I went to a doctor after two months and he recommended a urine test and an ultrasound. The urine test report was normal but ultrasound showed little infection on my testis and epididymitis. The doctor asked me to have Oflox-400 for 10 days and after that I had my ultrasound done again, which was absolutely normal. But now, I feel pain in my right testicle at times. I also experience a little irritation on the top of my both testis. I wear a supporter daily.
A.  You apparently had trauma or infection resulting in swelling and pain in your testicle. Although inflammation settles quickly, swelling takes a long time, even up to months, to become normal. If your testis has become anatomically normal, you do not have much to worry. If the swelling or nodularity has persisted, you must see the physician again to exclude chronic causes of inflammation like tubercular epididymitis.

Q. I am 32 years old and had a small accident few years back in, which I received a few stitches on my right index finger. My finger works all well but since last few days I am experiencing acute pain in the lower half of the finger and some stiffness as well. I am not able to decide which doctor to visit - a skin specialist or a surgeon. Please advise a doctor and the reason for the pain.
A.  The fact that you had stitches in your finger at the time of injury suggests that possibly your injury was a significant injury. However, it is very unusual that you present symptom few years after the injury. You could be having morning stiffness and pain, which could get worst during cold weather or rainy season. This is quite common in hand injuries occur because of residual stiffness in the joints. All that means is regular exercises to minimize the residual stiffness of your fingers. It is also possible that your current symptom is totally unrelated to your old injury. This can be distinguished only on careful clinical examination. You need to show yourself to an orthopaedic surgeon. Skin specialists are unlikely to be helpful in this regard.




Should my daughter be injected with tetvac

Q. My 1.4 years old daughter had got the pulp of fingertip of right thumb detached after getting pressed by the hinge of wooden door. We took her to the emergency of a nearby hospital with the detached part in icy water. The wound was examined and an X-ray done. The detached part was stitched in its place. I was told that there is no injury to the bone or nail bed. I was also told to get the wound examined and dressed daily because if the detached part is found to be dead it had to be removed immediately. Then we took her to a plastic surgeon who told us that the wound should not be dressed. He prescribed an antibiotic, a painkiller – Ecosprine (20 mg). Please answer my following questions:-
If I do not get the wound examined daily, can the dead part cause 

damage to the remaining fingerIs tetvac injection requiredWhat are the chances of the pulp getting attached


A.  You should get the wound examined at least every alternate day. While missing a day in between will not cause harm, even if the detached part is becoming black, it is best to follow the advice of the doctor who has seen the wound. If your daughter is fully immunized, and I presume she is, she should not need tetvac. For small wounds tetvac does not need to be given more frequently than once in five years. However, since we don't take regular doses, doctors simply give a shot every time the patient gets a wound. Since your daughter is 1.4 years old, she has probably gone through the full immunization regime. A booster is not essential now, but there is no harm if she does get a booster. (If she has not been fully vaccinated in her first year of life, please do get full immunization!) The chance of successful healing of the detached part is about 30%, if it was completely detached. A lot depends on the degree of crushing that took place. What next? Just show to your doctor. If the part heals, fine. If it doesn't, the operated part will get infected. She will need regular dressings till all the infection is gone, and possibly later some small surgery. Contact a plastic surgeon or a hand specialist.


How can I prevent injuries while playing?

Q. How can we prevent injuries while playing? I am a 29 years old man and play badminton every day. For the last week my right hand’s wrist is paining a lot. How can I avoid these types of strains? Please suggest.
A.  Following are some general rules for injury prevention no matter what sport you play. While it is impossible to prevent every injury, research suggests that injury rates could be reduced by 25% if athletes took appropriate preventative action, including:
Wear appropriate protective gear and equipment - Protective equipment that fits you well can save your knees, hands, teeth, eyes, and head. Never play without your safety gear.
Rest - People with high consecutive days of training, have more injuries. While many of us think the more we train, the better we will play, this is a misconception. Rest is a critical component of proper training. Rest can make you stronger and prevent injuries of overuse; fatigue and poor judgement
Always warm up before playing - Warm muscles are less susceptible to injuries. The proper warm up is essential for injury prevention. Make sure your warm up suits your sport. You may simply start your sport slowly, or practice specific stretching or mental rehearsal depending upon your activity.
Avoid playing when very tired or in pain - This is a set-up for a careless injury. Pain indicates a problem. You need to pay attention to warning signs your body provides.
Be in proper physical condition to play a sport - The weekend warrior has a high rate of injury. If you play any sports, you should adequately train for that sport. It is a mistake to expect the sport itself to get you into shape. Many injuries can be prevented by following a regular conditioning program of exercises designed specifically for your sport.

Q. What is the minimum time within which one should get Anti-tetanus injection after a small injury?
A.  An Anti-tetanus serum is not required in routine injuries. It is only recommended in an intensively crushed and dirty wound, in which case it is given within the first six hours after the injury. A routine tetanus toxoid is also not necessary if one is previously fully immunised. In case one is not, then the toxoid is administered within the first 24 hours after the injury.

A.  Given that the needle was old and the HIV virus is very fragile when exposed to air there is no meaningful risk of HIV infection. The risk of local infection has now passed. A tetanus toxoid injection would not go amiss but again the risk of contracting tetanus in these circumstances is negligible if at all.

Q. Dear sir,I met with an accident while driving my honda motor cycle... suddenly a dog came inbetween & felt but while falling i felt unconcious till the whole thing happens now I have a very small notch on my foot which I suspect might be DOG bite . I request you to please enlighten me if there is any check to confirm the dog bite.Also if so and I dont take any vaccine than what is the reaction . supposing there is no dog bite is it safe to still take rabbis injection ?? please advise. My local doctors have said there is no way to check dog bite also there is no reaction if you still take rabbis injection and it is not risky & nonreactive. pls. confirm whether it is correct.
A.  There is no way to check if it is a dog bite. The most important measures are to irrigate the wound thoroughly and take oral antibiotics for five days.If the dog can be traced and found to be healthy its not rabid. The rabies vaccine is unpleasant but not usually harmful unless there is a history of allergies.
